The absolute number not important (like 104F). It’s “OKAY” to have a fever of 105F. Give Tylenol and or Motrin. Go the ER for lethargy, significant shortness of breath, or dehydration, not because of the number on the thermometer. Your parental instinct is most important. If your gut tells you to go to the ER, then go to the ER. Parents often know best.

Category of its own - Tylenol (acetaminophen). It is NOT an anti-inflammatory. It can be given at the same time as NSAIDs. It works well for fever and headache, but not great for muscle pain. Adult Max dosing: 1 gram (2 extra strength 500 mg tabs OR 3 regular strength 325 mg tabs 4 times per day.) The max is 4 grams (4000 mg) daily.
NSAIDs - Non-Steroidal Anti-Inflammatory Drugs: These drugs bring down fever, reduce pain, and reduce inflation. They work well on macular pain. They cannot be taken in combination with other NSAIDs but can be taken with Tylenol even at the same time. NSAIDs taken long term, or sometimes even short term, that can irritate your stomach and cause an ulcer or bleed. They can affect your kidneys so after months, periodic lab maintenance is required. Advil/Motrin (ibuprofen): max dose: 800 mg three times per day (2400 mg daily.) Aleve (naproxen): Max dose 500 mg twice a day. Volateren (diclofenac 75 m): 75 mg- 1 tablet twice a day. Mobic (meloxicam 15 mg): 1 tablet 15 mg daily. Celebrex (celecoxib 100 mg): 1-2 tabs twice a day.
Steroid Ant-inflammatory Drugs (prednisone, methylprednisolone, hydrocortisone:) Should not be on long term. Only short term for many different reasons.

GENERALLY, Suspected Fracture don’t always have to be seen the same day, and can be evaluated he next day if:
Pain is controlled through stabilization on the injured area often with a splint.
Blood flow does not appear to be disrupted. (There is good coloring near the injured area)
There is no obvious neve damaged. (Normal sensation and general function).
Nothing appears crazy. Parents know best.

Sudden significant vision changes, vision loss, double vision, or missing portions of your visual field warrants and urgent ER visit.
You only have 2 eyes.

Chest pain that is worse with exertion (walking or exercising), nausea or gastro symptoms (vomiting/abdominal pain/diarrhea, sweating, or shortness of breath are concern because cardiac chest pain often presents differently in women and diabetics. This warrants and urgent ER visit.
You should call 911 or go to the emergency room immediately if you have chest pain that is intense, prolonged, or accompanied by other symptoms:
Typical cardiac chest pain presents as sudden, crushing, squeezing, or pressure in the middle of your chest that radiates to your jaw, left arm, back, neck, or shoulders

You should go to the emergency room (ER) for a headache if it's accompanied by other symptoms that are severe or could be a sign of a more serious condition:
Sudden onset - A headache that comes on suddenly and is debilitating or interferes with your daily life
Severe pain - A headache that's worse than any you've ever had, or that peaks in intensity within seconds
Neurological symptoms - Weakness, numbness, slurred speech, blurred vision, confusion, or stroke-like symptoms
Other symptoms - High fever, stiff neck, trouble walking, nausea or vomiting, or loss of consciousness

Certain strokes can be treated and potentially reversed through tPA therapy if done within 4 hours of onset of symptoms. There for treatment of stroke is emergent and timely.
Stroke symptoms include:
Sudden numbness, weakness or paralysis and drooping of the face, arm or leg, especially on one side of the body.
Suddenly blurred or decreased vision in one or both eyes.
Slurred speech, difficulty speaking or inability to understand or be understood.
Loss of balance or coordination.
